You can use this template to ask any question to your customers that can be answered by a 1 to 10 grader. For example, you could ask them how satisfied they are with your customer service or how likely they would recommend your company to others.

A great addition to this template is that you can select a threshold between satisfied and unsatisfied customers. For example, you could say that every customer who answers 5 or less is considered unsatisfied.

If a customer is considered unsatisfied, a text box will open so they can elaborate on why they are unsatisfied. That way, you can get direct feedback from your customers that you can use to improve.
